Does Media alt text property setting pull from Item's metadata, or Media's metadata?

On one of our soon-to-be-published projects, we have the Global Setting Media alt text property set to Dublin Core: Title, but I noticed that all of the images/Media attachments on our site are still missing alt text on the front end.

I wonder if I was misunderstanding this config setting. When selected, does this pull from the Dublin Core Title of the Media itself, or the Dublin Core title of the Item it’s attached to? I was kind of hoping for the latter, since the Media and Item will just have the same title across the board for this project, but I’m wondering if it’s actually the former…

It is pulling from the Media metadata:

Media alt text property: Select a field (for example, dc:title or dc:description) from the media description to use as alternative text if no alt text is explicitly set. Alt text is used in lieu of images (including thumbnails on browsing and search results pages) when a user is employing a screen reader and is an important part of meeting accessibility expectations.
