Shortcode for Simple Contact?

I’m trying to add a simple contact form within a text block on an exhibit page. It looks like there’s shortcode for this but I can’t seem to get it to work.

I’ve enabled the plugin in Omeka, and I’m typing the shortcode as: [simple_contact]
What am I doing wrong?
